If you require any vaccinations relating to foreign travel you need to complete a Travel Form. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. The practice requires at least twelve weeks notice before you travel.
Travel Vaccinations are not included in the NHS provision, however, some practices do provide them free of charge providing that you complete the necessary practice paperwork within the specified time limits
