Long Term Conditions Management / Monitoring

The practices aim to review patients with long term conditions or chronic diseases (eg asthma, thyroid, COPD, Kidney disease, diabetes, pre-diabetes, heart failure, cardio vascular disease, hypertension, osteoperosis etc) at least every 12 months.  Patients will be called into practice via invitations.  Appropriate advice and treatment are given where problems arise and referral to specialists will be arranged when required. You will normally be called in for review by text massage or telephone.  We will offer you 3 invitations, after which we will assume you do not wish to be reviewed, however it is important for your health that you do make an appointment.

Child Health Checks & Vaccinations

A child health and development service is offered to all children under five. Your child will receive invitations to attend for check-ups with the doctor  / vaccinations with the Practice Nurses periodically and it is most important for the health of your child to attend. Information about you or your child’s immunisation requirements can be obtained from the practice nurse or from your health visitor.

Travel Advice & Vaccinations

Our practice team is able to give a wide range of travel health advice as well as some travel vaccinations. Please make an appointment AT LEAST twelve weeks before you intend to leave the country.  The practice will not be able to undertake the vaccinations without sufficient notice.  You will need to complete a form to ascertain your vaccination needs.

Cervical Smear Tests

The practice operates a call and recall service for women aged 25 to 64. They will be sent an invitation to have a smear every three to five years (depending on age and medical history). Please specifically request an appointment for a smear test so we can make the necessary arrangements. You should receive the result of your test within 3-4 weeks. If you have concerns or the result appears delayed please contact our reception team. Patients with abnormal results will be contacted so that the doctor or nurse can explain what is wrong and any necessary follow up tests or treatment can be arranged.
